Licensed nurses collaborate with doctors to create a personalized Plan of Care that addresses the healthcare needs of each individual. They schedule regular visits to educate the client about their own health and to assess the progress made towards the treatment goals set by the healthcare team.

Physical Therapists (PT)
Physical therapists are licensed professionals that help improve movement and manage pain caused by illness, disability and/or injury. They are essential in the rehabilitation, treatment and prevention of patients with chronic conditions.

Occupational Therapists (OT)
Occupational therapists are licensed professionals that help patients overcome challenges that make independent living difficult. Their role is to help individuals recover, develop and maintain the skills necessary for daily functioning that have been affected by illness, disability and/or injury.

Speech Therapist (ST)
Speech therapists are licensed professionals that diagnose, treat and help prevent communication and swallowing difficulties. They are important in cases where the patient’s ability to communicate has been affected by various medical conditions (e.g. stroke, Parkinson’s disease, deafness, etc.).

Medical Social Workers (MSW)
Medical social workers are licensed professionals who specialize in advocating for patients. They facilitate ways for clients to receive medical care, find appropriate programs/facilities, obtain transportation and pay for their healthcare costs.

Home Health Aides (HHA)
Home health aides are professionals that assist ill, disabled, and/or injured individuals with daily living activities. Unlike caregivers, HHAs are licensed and have the ability to provide basic medical services (e.g. checking vitals, giving medications, etc.) under the supervision of a nurse.

We work hard to help reduce your fall risk, improve your ability to ambulate and prevent injuries that can result in stays at hospital/nursing homes.

IV Infusions and Injections
Most of our nurses are qualified to administer intravenous medications that require the use of IV lines and injections.

Wound Care and Management
Our team includes nurses who received additional training to treat wounds caused by injuries and other illnesses (e.g. diabetes, etc.).

To receive home health services, you must:
Be under the care of a physician
Be certified by the doctor to require intermittent nursing and/or other therapeutic (e.g. physical, occupational, and/or speech therapy) services
Be certified by the doctor as “home-bound”
(i.e. having difficulty leaving home without help due to illness or injury)Have Medicare, Medicaid and/or other selected private insurance
